About the Role | Mō te tūranga
This is more than a case management role. As a Disability Outcomes Coach, you’ll walk alongside people who self-manage their disability funding and employ their own staff. You’ll simplify complex systems, build confidence, and empower individuals and whānau to take control of their support journey.
You’ll embed the
Enabling Good Lives principles into everyday practice and help turn goals into achievable, measurable outcomes.
If you bring lived experience of disability (personally or within your whānau), or strong advocacy experience within the sector , we would genuinely value your perspective.
What You’ll Do
You will:
- Manage a caseload of Individualised Funding (IF) and Younger Persons with Disabilities (YPD) clients and families
- Coach individuals to confidently self-manage their funding
- Support the development and review of personal outcome plans
- Provide clear guidance on purchasing rules and eligible claims under Whaikaha – Ministry of Disabled People
- Celebrate progress and milestones with the people you support
- Maintain accurate records that reflect the journey and outcomes achieved
What You’ll Bring | Ō Pūkenga
- Experience in disability, health, or client-focused support services
- A strong belief in self-determination and empowerment
- Excellent communication and active listening skills
- Confidence navigating systems and explaining complex information clearly
- Solid computer skills and database experience
- A collaborative mindset with the ability to work autonomously
- A full NZ driver’s licence and legal right to work in New Zealand
