This role also serves as an internal case manager and advocate for clients in the Habilitation program.</p> <p class="wnfdntf _1ibi0s3f5 _1ibi0s3ce _1ibi0s3ea"><strong>Salary: starting at $47k with quarterly performance-based incentives eligibility</strong></p> <h2>Key Responsibilities</h2> <h3>Client Care and Case Management</h3> <ul> <li> <p class="wnfdntf _1ibi0s3f5 _1ibi0s3ce _1ibi0s3ea">Participate in person-centered planning processes and collaborate with clients, families, case managers, Habilitation Technicians, and interdisciplinary team members to implement and evaluate client care needs and individual goals.</p> </li> <li> <p class="wnfdntf _1ibi0s3f5 _1ibi0s3ce _1ibi0s3ea">Perform admission intake, when applicable, based on client criteria and training requirements.</p> </li> <li> <p class="wnfdntf _1ibi0s3f5 _1ibi0s3ce _1ibi0s3ea">Complete regular client visits to evaluate progress, monitor service delivery, and ensure services are implemented in alignment with the Person-Centered or Individual Support Plan and service definitions.</p> </li> <li> <p class="wnfdntf _1ibi0s3f5 _1ibi0s3ce _1ibi0s3ea">Maintain ongoing communication with case managers and care coordinators and ensure complete, accurate, and timely documentation in the client chart, home chart, and computer system.</p> </li> </ul> <h3>Field Staff Leadership and Supervision</h3> <ul> <li> <p class="wnfdntf _1ibi0s3f5 _1ibi0s3ce _1ibi0s3ea">Provide oversight and supervision to Habilitation Technicians in accordance with BAYADA policy, procedure, and state regulation.</p> </li> <li> <p class="wnfdntf _1ibi0s3f5 _1ibi0s3ce _1ibi0s3ea">Participate in hiring, interviewing, orientation, training, coaching, recognition, counseling, and performance feedback for field staff.</p> </li> <li> <p class="wnfdntf _1ibi0s3f5 _1ibi0s3ce _1ibi0s3ea">Serve as a liaison between field and office staff to support coordinated, high-quality care.</p> </li> </ul> <h3>Office and Program Support</h3> <ul> <li> <p class="wnfdntf _1ibi0s3f5 _1ibi0s3ce _1ibi0s3ea">Support office teammates, participate in performance improvement activities, and represent BAYADA in community and marketing efforts.</p> </li> <li> <p class="wnfdntf _1ibi0s3f5 _1ibi0s3ce _1ibi0s3ea">Assist in the development, management, and implementation of Habilitation services and contracts while maintaining positive relationships with referral and funding sources.</p> </li> <li> <p class="wnfdntf _1ibi0s3f5 _1ibi0s3ce _1ibi0s3ea">Provide guidance to office colleagues to promote understanding of and responsiveness to the needs of clients with intellectual and developmental disabilities and related conditions.</p> </li> </ul> <h2>Qualifications</h2> <ul> <li> <p class="wnfdntf _1ibi0s3f5 _1ibi0s3ce _1ibi0s3ea">Demonstrates the characteristics of The BAYADA Way: compassion, excellence, and reliability.</p> </li> <li> <p class="wnfdntf _1ibi0s3f5 _1ibi0s3ce _1ibi0s3ea">Prior supervisory experience required.</p> </li> <li> <p class="wnfdntf _1ibi0s3f5 _1ibi0s3ce _1ibi0s3ea">Demonstrated record of goal achievement, strong interpersonal skills, and success in taking on increasing responsibility.</p> </li> <li> <p class="wnfdntf _1ibi0s3f5 _1ibi0s3ce _1ibi0s3ea">Competence in basic PC skills and the ability to read, write, and communicate effectively in English.</p> </li> <li> <p class="wnfdntf _1ibi0s3f5 _1ibi0s3ce _1ibi0s3ea">Must successfully complete all required pre-employment assessments and background checks in accordance with BAYADA and state requirements.</p> </li> </ul> <h2>Hawaii-Specific Requirements</h2> <ul> <li> <p class="wnfdntf _1ibi0s3f5 _1ibi0s3ce _1ibi0s3ea">Must have a bachelor’s degree from an accredited college/university or Positive Behavior Supports training provided or certified by the Hawaii Department of Developmental Disabilities.</p> </li> <li> <p class="wnfdntf _1ibi0s3f5 _1ibi0s3ce _1ibi0s3ea">Must complete continuing education and/or training in Positive Behavior Supports at least once every two years.</p> </li> <li> <p class="wnfdntf _1ibi0s3f5 _1ibi0s3ce _1ibi0s3ea">In Hawaii, a Habilitation Manager may serve as a qualified supervisor.</p> </li> <li> <p class="wnfdntf _1ibi0s3f5 _1ibi0s3ce _1ibi0s3ea">Must perform face-to-face observation and supervision at least monthly, or more frequently as indicated in the client’s ISP.
