Help Turn Data into Better Patient Care

At Butte Home Health & Hospice, we believe every patient deserves exceptional care—and every improvement begins with understanding the data.

We're looking for a Quality Data Analyst who enjoys solving problems, identifying trends, and transforming information into meaningful insights. This is an exciting opportunity to help shape the future of one of Northern California's leading nonprofit home health and hospice organizations.

If you love finding patterns, building dashboards, asking "why," and helping organizations improve, we'd love to meet you.

About the Role

The Quality Data Analyst supports our Quality Assurance and Performance Improvement (QAPI) program by collecting, analyzing, and presenting clinical and operational data across our Home Health and Hospice programs.

This position serves as a strategic partner to clinical leadership by helping identify trends, monitor quality indicators, and provide actionable information that improves patient outcomes and organizational performance.

Rather than simply reporting numbers, you'll help tell the story behind the data.

What You'll Do

Data Analysis & Reporting

  • Design and maintain quality dashboards and scorecards
  • Analyze trends in Home Health and Hospice quality measures
  • Prepare reports for leadership, QAPI committees, and regulatory requirements
  • Ensure data accuracy and integrity across reporting systems
  • Monitor key performance indicators and identify emerging trends

Clinical Performance Analytics

Track and analyze measures such as:

  • Hospitalizations
  • Emergency Department utilization
  • Falls
  • Infection trends
  • Wound outcomes
  • OASIS quality measures
  • Home Health Value-Based Purchasing (HHVBP) metrics
  • Home Health Star Ratings
  • Hospice quality measures (HOPE and CAHPS)
  • Visit utilization
  • Length of Stay
  • Live Discharges
  • Other clinical and operational quality indicators

Decision Support

  • Identify opportunities for improvement using clinical and operational data
  • Prepare visual dashboards and presentations for leadership meetings
  • Help prioritize areas requiring further investigation
  • Support Performance Improvement initiatives by measuring baseline performance and tracking results over time

Technology & Process Improvement

  • Develop automated reports and dashboards whenever possible
  • Improve reporting efficiency and reduce manual data collection
  • Explore new ways to visualize and communicate quality data
  • Assist with special analytical projects that support organizational goals
